Essential Responsibilities: Tracking preventative care requests. Works with members and CMs to connect members to preventative care providers and schedule appointments. Registering members for vaccine appointments with providers, in community and home Troubleshooting issues member experience with medical providers and appointments Requesting and gathering needed medical documentation Assisting with Person Centered Service Plan administrative tasks Coordinating member outreach and care management mailings Provides administration and department support for care management team. Assists in completing activities related to different projects/initiatives in support of departmental goals. Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed. Performs other duties as assigned. Specific Knowledge, Skills, and Abilities: Proficient in the use of office equipment and the computer; Microsoft Word and Excel. Self-directing, good organizational and interpersonal skills. Required Education, Experience, and Licenses: Graduate of high school. 1 year satisfactory office experience; experience in Health Care, desired.
