How to Create a Fillable Form in Word

A fillable Word form uses real content controls: text fields, checkboxes, and drop-downs that people can fill without disturbing the surrounding layout. The tools are on the Developer tab, which is hidden by default.

Use this for Word for Microsoft 365, Word 2021, and Word 2019 on Windows or Mac when making a form, questionnaire, intake sheet, checklist, or template that others should fill out cleanly.

Turn on the Developer tab

This is a one-time setup.

  1. Open File > Options on Windows, or Word > Preferences on Mac.
  2. Choose Customize Ribbon, or Ribbon & Toolbar on Mac.
  3. Check Developer in the main tabs list.
  4. Click OK or Save.

The Developer tab now appears on the ribbon with form control tools.

Add form fields to the document

Type the form labels first, then insert controls.

  1. Type labels such as Name, Email, Department, or Preferred contact method.
  2. Click where a field should go.
  3. Open the Developer tab.
  4. Use Rich Text Content Control or Plain Text Content Control for typed answers.
  5. Use Check Box Content Control for yes/no choices.
  6. Use Drop-Down List Content Control for preset options.
  7. For drop-downs, click Properties and add each list item.
  8. Repeat until every field is placed.

Protect the form layout

Restrict editing after fields are ready.

  1. Open Developer > Restrict Editing, or Review > Restrict Editing in some versions.
  2. Under Editing restrictions, choose Filling in forms.
  3. Click Yes, Start Enforcing Protection.
  4. Set a password if needed.

More control

Customize placeholder text

Select a content control, open Developer > Properties, and edit the placeholder wording so users know what to enter.

Save as a template

Use File > Save As and choose Word Template (.dotx). People who open the template get a fresh form instead of overwriting the original.

Turn off Design Mode

Design Mode is for editing controls. If users see tags or fields do not behave like fillable items, click Developer > Design Mode to turn it off.
